What is StaleElementReference Exception?? How to get rid of it?

Hello Guys,
This video would help us understand the basic reason behind StaleElementReference Exception. Summarizing it:
StaleElement Reference Exception can happen due to following reasons:
1. The page itself got refreshed entirely due to some operations performed by you
2. Your code refreshed the page
3. (The most common one). The element/ its section ONLY got refreshed due to AJAX call which is not visible externally.
Solutions:
1. Re-initialization of element
2. Explicit wait to wait until element gets re-attached to DOM
