Latest 2021 Requirements for Package A, B, & C

Requirements for Package A Students:
1. 2 Photocopies of Family Card (KK)
2. 2 Photocopies of Birth Certificate
3. Completed Registration Form
4. 3x4 Photographs (4 sheets) and 2x3 Photographs (4 sheets)

Requirements for Package B Students:
1. 2 Photocopies of Family Card (KK)
2. 2 Photocopies of Birth Certificate
3. 2 Legalized Photocopies of Elementary School/Islamic Elementary School/Package A Diploma & SKHUN (2 sheets)
4. Completed Registration Form
5. 3x4 Photocopies (4 sheets) and 2x3 Photographs (4 sheets)

Requirements for Package C Students:
1. 2 Photocopies of Family Card (KK)
2. 2 Photocopies of Birth Certificate
3. 2 Legalized Photocopies of Junior High School/Islamic Junior High School/Package B Diploma & SKHUN (2 sheets) 2 legalized copies
4. Complete the Registration Form
5. 4 passport photos (3x4 cm) and 4 photos (2x3 cm)
