7 Days to Die alpha 20 has way too much new stuff to list. Seriously. Go read the patch notes, but make sure you're comfortable.
To be sure, though, check out the pipe weapons. The new pipe pistol, pipe shotgun, pipe machine gun, pipe shotgun, and pipe baton are a hoot. The Fun Pimps have outdone themselves this time around!
