On January 31, 2000, an Air Alaska plane flying from Puerto Valarta, Mexico, to San Francisco, California, suffered a mechanical failure during flight. The elevator trim tab jammed, and the plane crashed into the sea. All 88 people on board were killed. An investigation later revealed that the disaster was due to gross negligence on the part of the airline. Two years earlier, a mechanic had unsuccessfully requested the replacement of the faulty part that caused the crash.
