McOsu - Remote Control [Insane] AR -∞ EZ SS
I had a lot of mindblocks on this, was quite tough for me o.o I got 3 new AR -∞ scores today including this one, I'll upload the other 2 over the next couple of days so look out for those :D

McOsu is a practice client of osu! by McKay42 (https://github.com/McKay42/McOsu), which is similar to osu! but with more features
To make all the notes appear at the same time, go to the mod selection screen, hover your mouse over the left side of the screen to reveal additional mods and enable the Mafham mod (huge thanks to McKay42 for adding this! If you don't have this mod, make sure to use the Steam version of the game)
