The employer survey also shows a pattern of reduction in health care coverage offered to employees in smaller firms.
Approximately 99 percent of firms with 200 or more workers offer coverage.
Some employers are using wellness programs to encourage employees, occasionally with cash incentives, to take medical tests, exercise, and quit smoking.
A Professor at the University of Pennsylvania School of Medicine Kevin Volpp said: "We're seeing a big move in this direction driven by employers' concern about rising health costs and their sense that employee behavior has a lot to do with high costs."
One catch is that the programs cannot be mandatory.
