Reflex sympathetic dystrophy or Complex regional pain syndrome is a painful condition that is frequently confused with arthritis.
The prevalence and recognition of the condition appear to be increasing.
Severe extremity pain leading to nearly complete loss of function is the hallmark of the condition. Evidence of autonomic dysfunction is demonstrated by color changes, temperature differences, and dyshidrosis in the affected extremity.
Foot involvement is more common than hand involvement. A puffy swelling of the entire hand or foot is common. On examination, cutaneous hyperesthesia is evident to even the slightest touch.
Results of laboratory tests are negative.
Radiographic findings are normal except for the late development of osteoporosis.
Bone scans are helpful and demonstrate either increased or decreased blood supply to the painful extremity.
The cause of this condition is unknown.
Unlike adults, children only occasionally have a history of significant physical trauma at disease onset.
How autonomic dysfunction causes severe somatic pain is unknown, but the feedback cycle does provide the basis for treatment.
In mild cases, a program of rehabilitative physical therapy in combination with desensitization techniques will restore function and relieve pain.
