How to decrypt the floppy disk before launching Operation Chaos in Call of Duty: Black Ops Cold War

You need a 4 digit code and a passphrase to decrypt the floppy disk. You get the code by solving the anagram you get from the 1st evidence (Newspaper cutting). Solve the anagram by using the location names from the 3rd piece of evidence (Film reel). The number next to the location name is the code to decrypt the floppy disk. From the 2nd piece of evidence you will get a 4 digit number after you solve the red number and blue number series. This number will match with one of the numbers on the list from the 3rd piece of evidence(Film reel). The location name next to this number is the passphrase.
Note-The underlying logic remains the same, but every time you restart the campaign from scratch, the location name that you get after solving the anagram(got from the newspaper cutting) and number that you get from the 2nd piece of evidence (got after solving the red and blue number series)may change. So accordingly the code and passphrase will also change.
