How to Change Your Name in Oklahoma under the state statute 12-33-1631 that allows a resident to change their name after marriage, after divorce, or for any other reason that the petitioner may desire.
Name Change Petition. The name change petition in Oklahoma can be freely written, there is no official form. You must submit write or complete a sample form.
Notarized. You must have your petition authorized by a public notary. To do this, you must go to your local branch bank and they will usually have a notary on staff.
File Petition. Now you will need to file your petition with the county court clerk's office where you reside (See List of Courts).
Pay $159.00 filing fee.
Publish Your New Name. You will have to announce your name change to the public by publishing an ad in the newspaper that states your old and new name. The State does this, in case anyone would like to object to your new name.
After successful publication, be sure to collect the 'Proof of Publication' form. This will be provided by the newspaper.
Hearing. If someone objects to your new name, you will be scheduled a court date. If not, the clerk will notify you when your 'Name Change Decree' will be available.
Change Your Name. It is required that you change your name at all other government offices, and institutions you have relationships with.
