The U.S. Army would place an order for M16 rifles in late 1963, and designed it as the XM16E1. They opted for a forward assist, incase a round did not chamber correctly. Some of the things that caused a round to not chamber correctly could be crud, excess powder residue, soft chambering, cartridge issue, magazine issue, or a combination of those. It probably wont happen often, but I do know one recent instance where someone should have used the forward assist: A friend of mine heard hogs at night raiding his deer feeder and so he tried to hunt them, loading his AR15 rifle quietly. He went out there with a light, saw them, aimed and pulled the trigger but it just clicked. He realized he didn't ensure the round was seated correctly nor did he use the forward assist. He charged his rifle and aimed again but the hogs were on the run and out of sight. The hogs would come back some nights later, but this time knocking over his deer feeder, breaking it and eating all his corn. A neighbor finally trapped most of them, but by that time the damage was already done.

Note: Yes, I kept it dry for the purpose of the video. Also I rode the charging handle forward instead of letting it slam, a mistake some people make (like what my friend did in the description) but can be corrected by pushing the forward assist. Dirt and other matter could also not cause the bolt to fully seat in the chamber.
