Shortly after takeoff, ERA640, bound for Haneda Airport from New Chitose Airport, lost both engines.

Disasters are not accidental. They are the result of a chain of events.
Why did this catastrophe occur?

This time, I attempted to tackle the somewhat severe situation of a double engine outage.
