Hip Resurfacing vs Total Hip Replacement: Understanding Your Surgical Options
Considering hip surgery? Understanding the differences between #HipResurfacing and Total Hip Arthroplasty (Replacement) is crucial for making an informed decision about your treatment path. #shorts
What is Hip Resurfacing?
Hip resurfacing is a bone-preserving alternative to traditional hip replacement. Rather than removing the entire femoral head, HRA preserves more of your natural bone structure by simply capping the femoral head with a metal component while placing a metal socket in the acetabulum. This creates a metal-on-metal bearing surface that allows for a larger head size, which closely mimics natural hip biomechanics.
What is Total Hip Replacement?
Total hip replacement involves removing the damaged femoral head and replacing it with a metal stem inserted into the femur with a ball on top. The damaged socket (acetabulum) is replaced with a cup-shaped component. Various bearing surfaces are available, including ceramic-on-ceramic, ceramic-on-polyethylene, and metal-on-polyethylene.
Patient Selection: Who Benefits Most? Hip resurfacing shows excellent outcomes in specific patient populations:
Young males (under 55 yrs)
Patients with larger femoral head sizes (greater than 50mm)
Active individuals with high functional demands
Patients with primary osteoarthritis
Those wishing to return to high-impact activities
Total hip replacement remains preferable for:
Older patients (greater than 65 yrs)
Patients with poor bone quality or osteoporosis
Women planning pregnancies
Individuals with renal insufficiency or metal sensitivity
Patients with smaller femoral neck dimensions
Benefits of Hip Resurfacing include:
Superior bone preservation for possible future revisions
Impressive return-to-sport rates (studies show 92-98% return to previous activities)
Lower dislocation risk due to larger head size
More physiological gait patterns
Benefits of Total Hip Replacement can include:
Proven long-term success rates across diverse patient populations
Wider range of bearing surface options
Lower risk of metal ion concerns
More surgeon familiarity/wider availability
Better option for specific anatomical conditions
Long-Term Outcomes
Recent research involving over 11,000 hip resurfacing procedures showed outstanding survivorship in properly selected patients. A large international study reported 99% implant survivorship at 10 years and 92.5% at 21 years in male patients under 50. For comparison, the Swedish joint registry reported 89% survivorship for THA at 10 years in the same age group.
The decision between hip resurfacing and total hip replacement should be individualized based on your age, activity level, anatomy, and underlying condition. Consulting with an experienced orthopedic surgeon who performs both procedures is essential to determine the optimal choice for your specific situation.
For younger patients with high functional expectations and increased lifetime risk for revision, hip resurfacing offers an excellent bone-preserving intervention with low complication rates, impressive revision outcomes, and excellent return to activity potential.
