PS4 Bluetooth module or antenna problem (PS4 not detecting gamepad)

3) The gamepad intermittently disconnects from the PS4

If the gamepad doesn't connect to the PS4 at all and only connects in Safe Mode, you should try the following:
1) Replacing the Bluetooth module sometimes solves this problem.
