Identify the 2 sesamoid bones at the ball of the foot, under the joint where the big toe connects to the foot.
Pull the big toe back as far as it can go.
Using the thumb, compress just under the sesamoid bones.
With the big toe relaxed, push the big toe forward as far as it can go.
If pain is elicited, the test is positive for sesamoiditis.
