Can climbing bolts just fall out!? Here we do an in-depth failure mode analysis of a recent climbing bolt catastrophic failure. These are very rare but can happen, so let's see what might have happened and how it can be prevented in future. This was a flush head sleeve bolt and i show how to remove these in the last video. Usually, these are not possible to simply pull out, so the particular circumstances here contributed to the failure.
Climber & belayer suffered minor injuries but were back at the crag days later.
