Some of the team members you or your loved one might come across on the Skilled Home Health team include physical therapists, nurses, dietitians, occupational therapists, home health aides, speech-language pathologists, and medical social workers.
Lifespark Health Primary Care - senior savvy primary care that comes to you in the comfort of your own home. Our team tailors their primary care approach around you, your questions, your goals, and your wishes. You’ll have 24/7 access to your geriatric-experienced nurse practitioners and physicians, along with routine and additional visits as needed. No matter where you live, our medical support team will be there to keep your health on track.
