Today I’m going to explain whether open surgery, laparoscopic surgery, or robotic surgery is best for removing an appendix. Most people assume laparoscopy or robotic procedures are always superior—but let’s go over the facts. We'll compare all three based on anesthesia, incision, duration, cost, hospital stay, and postoperative pain.

Anesthesia Type
In open surgery, we usually use spinal anesthesia. A small injection is given in the lower back to numb the legs. The patient stays awake during the procedure.
But laparoscopic or robotic surgeries require general anesthesia. That means the patient is fully unconscious and a breathing tube is inserted. This is the first key difference.

Number of Incisions
In open surgery, we make just one cut—typically 4 to 5 cm on the lower right side of the abdomen.
In laparoscopic surgery, we insert instruments through 3 ports or small holes. These are definitely smaller than the open cut but more in number. Robotic surgeries also require multiple ports, with the robot controlling the instruments from outside.

Surgery Duration
An experienced surgeon can complete open appendectomy in 20 to 30 minutes.
Laparoscopic surgery takes more time. It demands more skill, and the setup adds extra minutes. Robotic surgery takes the longest due to machine setup and docking time.

Cost Comparison
Open surgery is the most affordable option.
Laparoscopic surgery involves costlier equipment and disposables, so it’s more expensive. Robotic surgery is the costliest of all, mainly due to the robot’s maintenance and consumables.

Hospital Stay and Recovery
Postoperative hospital stay is roughly the same for all methods—usually a day or two.
The main difference lies in postoperative pain. Laparoscopic surgery is less painful because of smaller incisions. Patients feel more comfortable in the first 2–3 days after surgery.
Pain and Infection Risk

Pain after open surgery depends on the size and location of the incision. If done by a skilled surgeon using a small cut, the pain is almost the same as laparoscopy.
Infection risk is the same in both surgeries, provided proper hygiene and sterilization are followed.

Conclusion
For uncomplicated appendicitis, all three options are safe and effective.
Laparoscopy offers better comfort, but open surgery is faster and cheaper. Robotic surgery has no added advantage in basic appendectomy. What matters most is who’s doing your surgery—not the tool in their hand. Skill always wins over technology in simple cases like this.
