How to create an interactive PDF form in Adobe® Acrobat®
Here is an outline of the steps used in this tutorial to create a fillable interactive form in Adobe Acrobat:
1. Open your PDF form in Adobe Acrobat
2. Select the Prepare Form icon (found in top menu: View, Tools, Prepare Form)
3. Select the Start button (check box if your form contains signatures)
4. While in Prepare Form mode, select a field to delete or add additional fields as necessary (Add a Text Field icon), drag to resize a field
5. Double-click on a field to rename and change other properties
6. Close button to exit the Prepare Form mode
7. Save form and distribute
