In this third video in his series on Breast Implant Illness (BII), Dr. Mackenzie begins the discussion of the surgical treatment of BII. Implant removal, done simultaneously with total removal of the breast implant capsule is called en bloc capsulectomy.
Complete removal of the implant capsule in BII patients is important to remove anything that may be triggering the systemic reactions. Silica is one example. Capsulectomies are familiar to plastic surgeons, however not all plastic surgeons treat BII patients and BII en bloc capsulectomies are more demanding. Their distinctions are discussed: En bloc capsulectomy, total capsulectomy, and partial capsulectomy.
Capsular contracture is a common complication of implants which every plastic surgeon is familiar with, and is a thick capsule that is relatively easy to remove. Ironically most capsules in BII patients are thin and supple and "normal" looking to the naked eye. Bacterial or fungal infections can be encountered, but these are rare in patients with no breast pain or swelling.
