This is an instructional guide on how to change your name in the State of Illinois under statute 21-101. This is valid for all name changes after marriage, after divorce, or for personal reasons.
Name Change Petition. - Once you have decided the name that you would like, you will have to fill out an Illinois Name Change Petition. This will be your official application to the State that you are requesting your name to be changed.
Find a Notary. - You will need to find a notary, you can find one at every branch bank in the United States.
File Petition. - You will need to file your name change petition to the county where you reside in Illinois.
Publish in Newspaper. - You will need to fill in the Illinois Notice of Name Change in a newspaper that circulates to over 100,000 people or publish in a law bulletin for 6 weeks. If you go to the county court clerk's office you will find the necessary office to publish the law bulletin.
After successful running of your ad, be sure to get the Notice of Publication.
Submit Your Forms. - You must go back and submit your forms again, with the publication of name change form, name change petition, and the Order for Name Change.
You will need to pay a $319 filing fee.
Attend Court Hearing. - You will need to go to your scheduled court hearing. Upon approval, you will authorized a judgement which you will need to fully authorize at the county court clerk's office.
