Use a form that you can send to your clients automatically. Then have that form feed into a spreadsheet where you can analyze the data and see if you are measuring up to the goals you set for yourself.
There are some great tools for forms out there.
Google forms is free and you can do some pretty complex stuff with it.
It's biggest limitation is that you are very limited in the design.
But otherwise it's a great tool to use. It also exports your responses to a spreadsheet that you can then analyze and manipulate.
A step up from that is Typeform.
This tool has a free option that's not bad. But it's limited to 6 questions and no question logic unless you upgrade - and then it's one of the best tools out there.
The upside to this tool is that you can design it quite a bit, which is great for branding. It gives a great user experience for your clients. And of course, it has some great integrations that are very useful as well.

There are now tools to create forms based on video. Responses can be written or in video format as well.
