Reporting an Incident
An Incident is defined as an unplanned event resulting in personal injury or property damage. WSO allows the user to record incidents and address them promptly by setting notifications / actions and follow-ups. This will in turn trigger an investigation and associated Corrective Actions and Preventive Actions (CAPA) for the incident. Personnel can be assigned to responsibilities and tasks to review the reported incidents. By identifying Incidents and their direct causes (unsafe acts or unsafe conditions) investigations can be assigned and CAPA performed to re-mediate the risk.
