I did a lot of mistakes here, from letting the worst enemies getting possessed to letting possessed enemies recover from GK state to using onslaughts either accidentally or at the wrong time (I don't have enemy spawns fully memorized), tho most of that is in the Holt; Cultist Base and Reclaimed Earth are fine. The run would've been very long regardless.
In traversal sections I obviously had to use Meathook.
Ignore the previous best, I probably got it by spamming console commands on a run, I don't remember when.
