I am an OBGYN resident, and this video is intended to briefly review the chemotherapeutic agent, Paclitaxel. This video is intended for medical students, residents, and fellows. Please enjoy!

Purpose and Mechanism of Action:
Paclitaxel is an antineoplastic agent primarily used in the treatment of various cancers, including ovarian, breast, and lung cancer. It belongs to a class of medications called taxanes and works by interfering with the normal function of microtubules, which are essential for cell division and growth. Paclitaxel stabilizes microtubules, preventing their disassembly during cell division and ultimately leading to cell cycle arrest and cell death.

Indications:
Paclitaxel is commonly used in the following gynecological oncology settings:

Ovarian Cancer: Paclitaxel is frequently used in combination with other chemotherapeutic agents as a first-line treatment for advanced ovarian cancer, both before and after surgery.

Breast Cancer: It is used in both early-stage and advanced breast cancer. Paclitaxel can be administered as a single agent or in combination with other chemotherapy drugs or targeted therapies.

Endometrial Cancer: Paclitaxel may be used in combination with other drugs for the treatment of advanced or recurrent endometrial cancer.

Administration:
Paclitaxel is typically administered intravenously (IV) in a hospital or clinic setting. The medication is formulated in a solvent, such as Cremophor EL (polyoxyethylated castor oil), which helps to solubilize the drug. Pre-medication with corticosteroids and antihistamines is often given to reduce the risk of hypersensitivity reactions associated with Cremophor EL.

Dosage and Schedule:
The specific dosage and treatment schedule of paclitaxel can vary depending on the type and stage of cancer being treated, as well as individual patient factors. It is usually administered every 3 weeks (once every 21 days) in combination with other chemotherapy agents. The dosage is determined by body surface area (mg/m²) and is calculated by the healthcare provider.

Side Effects:
Paclitaxel can have various side effects, and it's crucial for an OB/GYN resident to be aware of them. Common side effects may include:

Hematologic Effects: Paclitaxel can cause a decrease in the production of blood cells, leading to anemia, neutropenia (low white blood cell count), and thrombocytopenia (low platelet count).

Neurotoxicity: Peripheral neuropathy, characterized by numbness, tingling, or pain in the hands and feet, is a common side effect. Symptoms may improve or resolve after treatment completion.

Hypersensitivity Reactions: Although pre-medication is given to minimize this risk, hypersensitivity reactions can still occur, ranging from mild allergic symptoms to severe anaphylaxis. Close monitoring during administration is necessary.

Gastrointestinal Effects: Nausea, vomiting, diarrhea, and mucositis (inflammation of the mouth and gastrointestinal tract) can occur.

Hair Loss: Temporary hair loss, including scalp, body, and pubic hair, is a common side effect.

Precautions and Considerations:

Pregnancy and Lactation: Paclitaxel should be avoided during pregnancy as it can harm the developing fetus. It is also not recommended while breastfeeding.
Hepatic and Renal Impairment: Dose adjustments may be necessary in patients with liver or kidney dysfunction.
Drug Interactions: Paclitaxel can interact with other medications, so it's important to consider potential interactions and adjust dosages accordingly.
