The Family Caregiver Support Program (FCSP) at St. Barnabas Senior Services Program Manager, Amy Blackburn, is the presenter in this video. Amy graduated with a Master of Science in Gerontology from California State University, Long Beach and graduated with a Master of Public Health from University of Southern California.

Guest, Kayla Shore with Hand in Hand introduces a research study and survey on the long-term care industry in partnership with the UCLA Labor Center. She is encouraging family caregivers, or their care managers, to take the survey; those who hire and bring in an in-home paid caregiver to provide relief, respite, or a brief break for the family caregiver; or, if the family ended up needing to place a loved one in a residential board and care home (Residential Care Facility for the Elderly or RCFE), possibly due to caregiver burnout or other reasons.

Amy then transitions to the PowerPoint presentation and briefly discusses aging and common acute and chronic conditions that could occur in our twenties, thirties, forties, fifties, sixties, and so on; and how best to manage the accumulation and interaction of preventable and unpreventable physical and emotional difficulties. The Nagi model can be used as a tool, and act as a visual road map, showing potential pathways to disability. But we can be proactive with adding healthy lifestyle changes and specific interventions to prevent limitations and functional impairments from becoming permanent disabilities within our life roles; or at least slow the progression to have a better quality of life for as long as possible. High blood pressure or hypertension (HTN) is reviewed within the Nagi Model, as well as obesity and type 2 diabetes. Recognizing our standards for our quality of life and happiness, knowing which dimensions of wellness are important, and setting S.M.A.R.T. goals can help us age optimally; especially with unexpected, stressful life events. Family Caregivers have seen firsthand how loved ones lose independence, become disabled, and need assistance. So, they have a better ability to recognize how to prevent the need for long-term care or needing a caregiver, themselves. This video provides the tools caregivers need to achieve that. Love 2 Care is sponsored by the City of Los Angeles Department of Aging.
