"Designing Value-Based Payment (VBP) Innovations for Pediatric-to-Adult Transitional Care: Lessons from the Field"
This webinar is intended to assist state Medicaid agencies, commercial insurers, health plans, and pediatric and adult practices to expand their value-based efforts around pediatric-to-adult health care transition. It does so by 1) discussing a step-by-step approach to designing value-based payment (VBP) innovations for pediatric-to-adult transitional care, and 2) hearing from senior officials in two large Medicaid MCOs - Florida Department of Health Children’s Medical Services Health Plan (operated by Sunshine Health) and Texas AmeriGroup – about their own experiences with designing a transition VBP pilot.
The webinar was hosted by The National Alliance to Advance Adolescent Health/Got Transition on 1/26/22 and was made possible with funding from the Lucile Packard Foundation for Children’s Health.
