Sorry this video is wasn't recorded in real-time. I can post a video of how I corrected this bug if anyone is interested or you can read the fix below.
The issue it was having was when I duplicated a subtool (the fingers) the copy shrinks really tiny to the center of world coordinates. I tried restarting Zbrush, checking mesh integrity and trying it again but it still had issues. I'm sure the project or tool got corrupt and was embedded on that particular subtool.
I was able to correct this issue and duplicate the subtool with the correct size/scale if I cloned a copy of the subtool as a new tool not in the same project. I then opened up a new project and appended all the subtools (separately or merged) into the new project and the duplication feature worked correctly again.
