Nice bug, plarium. (READ DESCRIPTION)
So i noticed my duchess hadn't proc'd reaction in a while, so i decided to see and watch how long it'd take for her to proc it... turns out in 39 ish 3v3 tokens, where she'd get crit 1-3 times (like this video) not once had reaction proc'd on her, while wearing... THREE, THREE reaction accessories. nice 1 plarium, super cool.
