Gynecomastia Demystified: Grading and Treatments

Gynecomastia is a common condition that affects many men around the world. To assess the severity of gynecomastia, most doctors use a grading system, which categorizes the condition into four grades. These grades range from mild to extreme, taking into account the size and amount of sagging of the breast tissue.

Grade 1 is Mild gynecomastia. This is slight breast enlargement and no sagging. An example would be a body builder with puffy nipples. Treatment for this may require gland excision through a small infra-areolar incision plus or minus liposuction.

Grade 2 is Moderate gynecomastia, with moderate breast enlargement and slight sagging. Treatment for this may require gland excision through a small infra-areolar incision plus or minus liposuction with or without the use of skin tightening devices such as Renuvion. This is also accompanied by the use of a compression garment in the post operative phase.

Grade 3 is severe gynecomastia, with significant breast enlargement and sagging. Treatment for this may require gland excision through a infra-areolar incision plus or minus liposuction with the use of skin tightening devices such as Renuvion. Sometimes depending on the quality of the skin, a formal breast reduction may be required. This is also accompanied by the use of a compression garment.

Grade 4: Extreme Gynecomastia or a man with extremely large and pendulous breasts, to the point where the tissue hangs down several inches and is visibly noticeable. The treatment for Grade 4 gynecomastia often requires a full breast reduction procedure. This is because the excess skin after surgery may be past the point of what can be achieved with renuvion. Additionally, the use of a compression garment is usually recommended post-surgery to help support the treated area and aid in healing.

While non-surgical treatments, such as maintaining a healthy diet and exercise regimen, can help to decrease the size of the gland, they will not eliminate it. Exercise and weight loss can be effective in reducing the size of the gland, but the definitive treatment for gynecomastia is surgery.
