This is a 11 year old game The Elder Scrolls V : Skyrim SE / AE - Special / Anniversary Edition with many months, equivalent of 1500+ hours, spent ultra modding in attempts to modernize it. Skyrim modded to feel like Elden ring / Soulsborne / Dark Horror fantasy game.
