In a dimension far removed from the world as we know it, Bob the Barbarian awakens to an unusual sensation: something wriggling in his skull. The room around him is filled with pulsating, alien-like chambers. As he stumbles to his feet, the world swirling around him, he realizes he's aboard a Nautiloid – a ship belonging to the malevolent Mind Flayers.
Peering into a nearby pool, Bob's reflection shocks him: an alien tadpole has been implanted into his eye, slithering its way deep within his brain. It's clear he's been infected, and the barbarian's natural instinct to smash anything uncomfortable isn’t going to help him here.
As the Nautiloid navigates the chaotic astral sea, Bob encounters other unfortunate souls also implanted with these tadpoles. Among them are a haughty Elven cleric, a cunning Tiefling rogue, and a curious Githyanki warrior. Despite their vast differences and initial suspicions, they all share a common goal: remove the parasites and escape the Mind Flayer's grasp.
The ship is suddenly attacked by red dragons ridden by Githyanki knights. As the two colossal forces battle, the Nautiloid's emergency protocols are triggered. In a desperate escape attempt, the ship teleports – but not without consequences.
Bob finds himself, along with his newfound allies, crashing outside the city of Baldur's Gate. As they take their first breaths in this new environment, they realize the tadpoles aren’t just any parasites: they're the seeds to turn them into Mind Flayers. The clock is ticking, and Bob's barbarian strength won't be enough. They need a cure, and fast.
The group decides to venture together into the dangers of Faerûn, battling not only external foes but the very alien nature growing within them. With Baldur’s Gate on the horizon, Bob the Barbarian’s journey has only just begun...
