In this video, Dr. Eric Rulli shows what cannot be missing from the initial prescription to quickly reverse discomfort, safely reduce afterload, and avoid unnecessary intubation. Practical focus: when to choose nitroglycerin or nitroprusside as titration, when to start furosemide, and what pressure targets are needed in the first few hours.

✔ Recognize the hypertensive phenotype and differentiate it from pulmonary edema in normotensive or hypotensive patients.

✔ Continuous monitoring: Serial blood pressure checks every 10 minutes, oxygen saturation, urine output monitoring, and criteria for bladder catheterization.
✔ Pitfalls to avoid: starting with diuretics alone, abruptly reducing blood pressure, using glucose solution without indication, and neglecting analgesia for respiratory discomfort.
✔ Identify and treat triggers: poor adherence to antihypertensive medication, salt and alcohol overload, infection, arrhythmias.
✔ Useful tests on call: complete blood count, urea and creatinine, electrolytes, chest X-ray when available.
