At Community Care Plan, it’s our goal to make sure that the voices of our community are heard loud and clear. As the health plan with a heart, we don’t just want to improve health care. We also want to improve everything that can affect it.
In this series, we'll discuss health, housing, food, safety, and money. All of the things that can impact your access to care, the quality of the care you get, and more.
It is our hope that with our Voices of the Community video series, we can serve as a voice for the people impacted the most by health care inequity.

From the start.
Housing.

Money.
We are aware
Of all these things
That can impact your access to care,
And the quality there.
With our Voices of the Community video series,
It is our intention
To alleviate
The tensions
For the people
Impacted most
By health care inequity
And the factors
That do not get mentioned.
Like the mother
without maternity care,
Who is not equipped.
Like the neighbor
who needs behavioral health services.
Like the family
that needs help finding a safe and affordable place to live.
The voices
Community Care Plan wants to give.

Shine a light to help us see
