This ipsilateral DRG-S lead insertion technique was designed to avoid transection of the paraspinal muscle and in turn prevent lead fracture. In this case first a midline incision was made prior to placing the leads and leads were anchored to deep fascia. Fixation by anchoring is recommended to prevent lead migration, which has been shown to significantly decrease lead migration, up to as low as less than 1% per lead. This more midline, ipsilateral approach bypasses the musculature which can ensnare the lead, cause friction, and ultimately disruption of the lead.
