Desfesoterodine is a drug with urinary antispasmodic effects used in adults to treat symptoms associated with overactive bladder syndrome, such as increased frequency, incontinence, or urgency.
Its mechanism of action is the antagonism of muscarinic receptors, that is, it binds to these receptors and blocks their activation, which would cause contraction of the bladder's detrusor muscle and relaxation of the sphincter to promote urination.
Thus, desfesoterodine opposes these effects and is able to reduce the symptoms of overactive bladder.
In Spain, it is available in pharmacies as prescription-strength tablets for oral administration.
The starting dose is 3.5 mg in a single daily dose, which can be increased to 7 mg in a single dose if necessary at the discretion of the physician.
The full effect of treatment is usually observed between 2 and 8 weeks. Therefore, it is recommended to reassess the efficacy of each individual patient after 8 weeks of treatment.
Caution should be exercised in patients taking other antimuscarinic medications—for example, tricyclic antidepressants or some antipsychotics—as these may increase the drug's effects, leading to a higher risk of adverse reactions.
As a precaution, desfesoterodine is recommended to be avoided during pregnancy and breastfeeding, and it is also contraindicated in patients with severe liver impairment.
The most common adverse effect associated with its use is dry mouth, which is related to its antimuscarinic effect. It can also cause dry eyes and throat.
Other side effects that may occur during treatment include gastrointestinal discomfort and, less frequently, urinary retention and difficulty initiating urination.
