The Vagus Nerve (Cranial Nerve X) 🧠
Anatomical Course and Branches
The vagus nerve, the 10th cranial nerve (CN X), is the longest and most complex of the cranial nerves. It originates in the brainstem's medulla oblongata and extends from the head down through the neck, chest, and abdomen.
In the Head
The vagus nerve exits the skull through the jugular foramen and the glossopharyngeal (CN IX) and accessory (CN XI) nerves.
Within the cranium, the vagus nerve gives off the auricular branch, which supplies sensation to the posterior external ear canal and external ear. 👂
In the Neck
In the neck, the vagus nerve travels inferiorly within the carotid sheath, alongside the internal jugular vein and common carotid artery. 💉
The right and left vagus nerves take slightly different paths at the base of the neck:
The right vagus nerve passes anterior to the subclavian artery and posterior to the sternoclavicular joint, entering the thorax.
The left vagus nerve passes between the left common carotid and left subclavian arteries, posterior to the sternoclavicular joint, entering the thorax.
Key branches of the vagus nerve in the neck include:
Pharyngeal branches - Provide motor innervation to the majority of the muscles of the pharynx and soft palate. 🗣️
Superior laryngeal nerve - Splits into internal and external branches. The external branch innervates the cricothyroid muscle of the larynx, while the internal branch provides sensory innervation to the laryngopharynx and superior larynx.
Recurrent laryngeal nerves - The right recurrent laryngeal nerve hooks under the right subclavian artery, while the left recurrent laryngeal nerve hooks under the aortic arch. Both nerves then ascend to innervate the majority of the intrinsic muscles of the larynx.
In the Thorax
In the thorax, the right vagus nerve forms the posterior vagal trunk, and the left vagus nerve forms the anterior vagal trunk. 🫁
Branches from the vagal trunks contribute to the formation of the esophageal plexus, which innervates the smooth muscle of the esophagus.

Left recurrent laryngeal nerve - Hooks under the aortic arch and ascends to innervate the larynx.
