This run is segmented by each save (so 4 segments in total). The whole thing is choreographed to look nice, but there are a couple of goofy-looking moments due to the length of the segments. This run includes all of the notes, and while I didn't take any special measures to make sure I collected every flashback, it seems I did trigger all of them by luck (according to the wiki, anyway), so I guess I 100%ed the lore.
