مريض في العشرينات جاى الطوارئ
ب صداع بقاله يومين"هيفجر دماغه" ومجالوش قبل كده بالشدة دي
تم قياس الحرارة كانت ٣٨.٢
المريض بيرجع وفقد القدرة انه يكلم
الفحص السريري وجدنا
manifestation of slight meningeal irritation
⭕تم عمل CT Brain لقيناها free مفيهاش حاجة
⭕تم عمل MRI
bilateral asymmetrical changes of the medial temporal lobes and inferolateral frontal lobes
⭕ تم عمل CSF examination
Lymphocytosis , elevated protein

⭕Most Likely Diagnosis ; Encephalitis
⭕Its Differential : Infectious causes (Viral is the most common cause) and it's viral here according to CSF
⭕Next Investigation is PCR For detection of the virus ( Which is HSV in this case....cause HSV encephalitis main affection site temporal lobe (lead to aphasia) and inferior frontal lobe
⭕If left Untreated ... Bad Prognosis with mortality rate 80% due to post encephalitis sequelae
⭕NOTES
⭐CT is normal ...cause CT is normal in 1\3 of the patients
⭐NOT Mainly Meningitis cause there's slight meningeal irritation with severe parenchymal involvement so Meninges is not dominant here but there's slight affection
⭐Other likely causes(but not the most likely here)...All DDx of encephalitis like Meningitis ,SAH ,Abscess
⭐CSF protein in viral causes is normal OR Mildly Raised
