We alight on a seaside village to learn all about congestive heart failure (CHF)--- and where better to evoke the pain and heartbreak of fluid retention than the timeless story of Little Mermaid. In this lesson, we break down the key differences between systolic and diastolic heart failure. We explore the causes of systolic heart failure, or heart failure with reduced ejection fraction, including coronary atherosclerosis, volume overload, and dilated cardiomyopathy. This volume overload leads to eccentric hypertrophy. On the other side, we look at diastolic heart failure, or heart failure with preserved ejection fraction, caused by afterload issues like hypertension and stenosis, leading to concentric hypertrophy. We also discuss how conditions like coronary atherosclerosis, hypertrophic cardiomyopathy, restrictive cardiomyopathy, and constrictive pericarditis make the heart less flexible. Finally, we delve into the vicious cycle of decreased cardiac output, compensatory mechanisms, and cardiac remodeling, which further worsens heart function.
