In this video guide I will show you how to find the secret in "Plan B". This can be found in Act 2, level called Hinterland.
This one can be found at the beginning of the level. Not too long after leaving the saferoom you will be walking in some shallow water until you jump off a small waterfall. Once you jump down turn around and inside the waterfall is the golden skull.
