Composite filling on left lower last molar. Deep distal cavity with gingival overgrowth
Before the case, an X-ray evaluation of the defect under the gingiva is crucial to see whether the defect is at the bone level; if so, the treatment plan might change. I removed the distal gingiva with a round steel bur and left the Clamp in place to protect the patient's tongue from the bur(I also asked her to lift up her tongue during the process for easier access).
Before placing the rubber dam, I also check each tooth contact with floss to see if any sharp area or overhanging restoration might tear the rubber sheet. During caries removal in a deep area, I also use a hand instrument to protect the rubber sheet in this area from the bur, just in case my hand slips 😅.
