Deep margin elevation + Ceramic overlay
In this case, the patient came with a lower left last molar that had already undergone root canal treatment. A few weeks prior, the mesial part of the tooth had broken and been repaired with a temp filling. The patient had discussed the treatment option with another dentist. The plan was to do crown lengthening. However, there was a risk of furcation involvement, or they might not be able to gain a lot of crown height due to anatomical limitations. So, the patient sought a second opinion.
After discussing treatment options, I did deep-margin elevation and cusp coverage with a ceramic overlay. Actually, endocrown is one of the options we discussed, but a shallow pulp chamber was presented from an x-ray, so I wouldn't be able to gain much retention from pulp chamber extension. Instead, I chose to preserve the enamel ring and use a ceramic overlay.
