The new fourth ending in Mass Effect 3 for the Extended Ending. Also includes the short clip that comes after the credits and the message from the Mass Effect Team
There are two ways to get this:
The way I did it. Shoot the Star Child.
Disagree with everything the Star Child says.

Reason behind the choice:
I originally intended to destroy the Reapers but I always has a dislike for the Star Child so I thought I would shoot him first. You know, just to vent my stress and try to "Flick the Bird" at him. I won't lie. He freaked me out when he said "SO BE IT!" Who would have known he knew what I was trying to say?
My Shepard was maxed out in Paragon points and also had an Effective Military Strength (EMS) of 4075.
