00:14 In the "10-digit Login Code" field, enter the code from the email message you received then press the green "Login" button...
00:21 If the displayed request details are what you expect, press the blue "Continue without account" button… If not, press the red “Decline ticket” button.
00:28 Complete/update the fields in the “Your details” panel as appropriate then press the blue "Continue" button…
00:35 The requested form will now be displayed onscreen, ready for completion…
00:42 When you have finished completing the form, press the green "Save" button at the bottom of the form. NOTE: As you have not logged-in, you will not be able to view or change this form after it is saved.
