00:07 Firstly, login to your NHS ePortfolios user account…
00:14 From the “Forms” menu, press the “Ticket Requests” menu item…
00:21 Press the green “Create Ticket” button to start to create a new ticket request…
00:28 The “Create Ticket” window will now open. Select the appropriate post if it is not automatically selected… NOTE: You must have either a current or recently finished post to create a ticket.
00:35 Press “Select Form” then choose the form you wish completed on your behalf…
00:42 Press “Select Recipient” then choose a supervisor / previous recipient or select the top item, entitled “Enter the recipients email…” if you have not requested a ticket from this individual recently.
00:49 If you chose “Enter the recipients email…” you will now be able to type the email address of the individual to whom you wish to send the request.
00:56 Next, add any comment you would like to accompany your request, being sure NOT to include any Personally Identifiable Information (PII).
01:03 If permissions allow you to pre-fill the form on behalf of the recipient and you wish to do so, ensure that the “I want to complete some of the form?” toggle is in the ON position before you click the “Create Ticket” button…
01:10 The ticket request has now been sent to the email address you selected / provided. Click OK to proceed…
01:17 If the “I want to complete some of the form?” toggle was in the ON position, the request form will now open and you will now be able to complete as much / as little of the requested form as you wish…
01:24 When you are finished pre-filling the form on behalf of the recipient, press the green “Save” button…
01:31 You can edit the pre-filled form content at any time before the form is completed by the recipient by pressing the “Complete some of the form” button beside the ticket request.
