As we all know, the vast majority of aviation fatalities start with bad decision making. Unfortunately, things get worse very quickly when poor judgement is repeated. But luckily, our industry is very well known for learning from the mistakes of others.

The scenario is a perfect example of numerous bad decisions compounding, leading to completely the avoidable deaths of the pilot and all of his passengers. There are many lessons to learn from this accident, whether you are a relatively new pilot, or have been flying for years.
