By the way, this only works on Windows, blame Apple for it.
MIDI Out uses Microsoft Synthesizer, and it seems like Apple doesn't like virtual hardware for "Security reasons".
MIDI Out does not make any sound of its own, it acts as a MIDI controller sending standard MIDI messages to internal VST plugins or external MIDI hardware.
