This patient is 78 years old and was referred to us from Izmir. A heart attack permanently damaged the area of ​​the heart where the mitral valve attaches. This resulted in a very serious leak in the valve. This meant that instead of all the blood flowing from the lungs to the heart being pumped back into the body, more than half of it began to leak back into the lungs. As a result, the patient's lung sacs, which should have been filled with air, filled with water, resulting in pulmonary edema, which led to intensive care admissions. Additionally, due to his history of partial paralysis and chronic kidney failure, he was considered a very high risk for heart valve surgery. We met the patient at this stage.

The day he presented to our clinic, he was again experiencing low-tension pulmonary edema. Therefore, with a very high risk of death, we admitted him to the intensive care unit and began treatment immediately. We also scheduled him for a procedure called MitraClip early on. This procedure uses a type of clip to secure the two opposing parts of the heart valve that are leaking blood. Depending on the situation, we insert one or more clips to minimize the leak as much as possible. We perform all of this non-surgically; that is, we complete the entire procedure without incising the rib cage and while the heart continues to beat. We insert the pawl-carrying system into the heart through a pencil-sized access port in the groin vein. This allows us to reduce mitral valve leakage in patients who are too high-risk for heart surgery and improve their deteriorating clinical picture. We can also perform the pawl-attaching procedure on the right side of the heart (tricuspid valve). In this case, we achieved a mild reduction in the severe leakage and discharged the patient two days later.
