My husband has 3% teratozoospermia. Karyotypes are normal, and the karyotypes of the pregnancies were normal. Now I'm not even getting pregnant. We've been treating him with high doses of Omega-3, Mexidol, and zinc. Selenium, vitamin D, arginine, and carnitine for six months. The cause of teratozoospermia is varicocele. They're not suggesting surgery because my husband is over 41. They say it could be even worse. We also had a CIR examination; we know the treatment during pregnancy.

Then there's varicocele. There's no such age limit. If there's a severe varicocele, venous congestion leads to toxic metabolites stagnating in the testicular tissue, which has a negative impact. There's also the thermal factor. That is, varicocele causes an increase in intratesticular temperature. Therefore, if sperm quality is reduced, surgery might be worth considering. But, again, regarding what kind of varicocele it is, it needs to be assessed individually. I always do this myself before accepting a patient for surgery. I've already talked about this. It's important to weigh the pros and cons, of course. But varicocele, if it's truly clinically significant, if it's palpable, and the vein diameter on ultrasound is appropriate, can also be a factor leading to impaired spermatogenesis. Therefore, it's essential to eliminate all preventable factors as much as possible.
Check the integrity of the genetic material. At least get a fragmentation test.
