Panelising gerbers for use at DirtyPCBs or other services

If you're getting PCBs made professionally, it may be possible to save money by making multiple designs, or multiple copies of a design at once in a single order or panel. But how do you create a panelised board from multiple gerbers? Here's how I did it with KiCad and GerberTools.
